Separated Endodontic Instrument: Retrieve, Bypass or Retain? | DentalReach
Forum/Endodontics

When should a separated endodontic instrument be retrieved, bypassed or retained?

ArticleRemoval of a Broken Instrument: An Endodontic ChallengeOpen

A separated instrument is not automatically a retrieval case. I weigh the stage of disinfection, fragment level, curvature, remaining dentine, periapical status and whether I can work under direct vision.

Which factor most often changes your decision?

For a structured risk-benefit pathway, read Broken Endodontic Instrument Removal: Clinical Guide dentalreach.today/removal-of-a-broken-instrument-an-endodontic-challenge

The potential gain from retrieval should be greater than the risk of perforation, excessive dentine loss or fragment extrusion.

Anatomy often decides the case. This three-canal premolar report shows why magnification and selective imaging matter: dentalreach.today/endodontic-management-of-a-mandibular-first-premolar-with-deep-split-into-three-canals-case-report

A fragment beyond a sharp curvature deserves a different plan from a visible coronal fragment.

The broader diagnostic and referral context is covered in Root Canal Treatment FAQ: Clinical Endodontics Guide dentalreach.today/frequently-asked-questions-root-canal-treatment-101

Patient disclosure, documentation and an outcome-based review plan remain essential even when the fragment is retained.
