ArticleEndodontic management of a mandibular first premolar with deep split into three canals - Case ReportOpen

In a mandibular first premolar, a sudden canal disappearance, eccentric file position or unexplained radiographic line should prompt a search for a split system. The challenge is finding anatomy without weakening the root.

Which clue has been most reliable in your cases?

The full case and a practical diagnostic table are here: Three-Canal Mandibular First Premolar dentalreach.today/endodontic-management-of-a-mandibular-first-premolar-with-deep-split-into-three-canals-case-report

I start with angled periapicals and magnification, then justify limited-field CBCT only if it will change treatment.

The FAQ connects this anatomy problem with missed canals, irrigation and referral: dentalreach.today/frequently-asked-questions-root-canal-treatment-101

A reproducible glide path and controlled irrigation matter more than forcing an instrument into a suspected branch.

If a file separates during negotiation, use the risk-based broken-instrument pathway: dentalreach.today/removal-of-a-broken-instrument-an-endodontic-challenge

Complex anatomy plus limited visibility is a sensible referral threshold.
