Cracked Tooth Syndrome: Diagnostic Dilemmas And Clinical Management
• Cracked Tooth Syndrome is often not a problem of treatment first—it is a problem of diagnosis first. The patient may know exactly when the tooth hurts, yet the clinician may struggle to identify which tooth is responsible.
• A normal periapical radiograph does not rule out a cracked tooth. Many cracks are too narrow, too favorably oriented, or confined to the crown to be visualized radiographically.
• The classic clinical clue is often pain on biting or, particularly, on release of biting pressure. However, symptoms may vary considerably according to crack direction, pulpal status, occlusion, and the extent of structural involvement.
