Root canal treatment should follow a pulpal and apical diagnosis based on history, clinical tests, periodontal findings and appropriate imaging. Symptoms alone are insufficient because cracked teeth, referred pain, occlusal trauma and non-odontogenic conditions can imitate pulpal disease. Reproducible findings and restorable prognosis should guide the decision.
