V. Mouth Breathing

Assess the child, not only the habit

Nonnutritive sucking is normal in infants and young children. Clinical concern increases when a habit persists with sufficient frequency, duration and intensity to influence the dentition or dentoalveolar development. Management should consider age, developmental readiness, airway, sleep, speech, emotional context and family expectations.

Structured habit assessment

  • Identify when, where and why the behaviour occurs and whether the child wants to stop.
  • Record overjet, open bite, crossbite, arch form, tongue posture and functional shift.
  • Screen nasal obstruction, snoring, sleep-disordered breathing and speech concerns for appropriate referral.
  • Distinguish an active habit from a residual malocclusion after the habit has stopped.

Stepwise management

Begin with non-judgmental counselling, positive reinforcement, reminders and achievable goals. Appliances are adjuncts for selected motivated children when conservative approaches are insufficient; they should not be punitive. Coordinate with paediatricians, ENT specialists, speech-language professionals or psychologists when broader factors are present.
